You should create the users over the web interface, not on the command line 
with sasl*.

Authentication is done over the (my)sql database.

In the default config, sending addresses are not verified.

Verify first that you have a working authentication over your database. Then 
if these errors still persist you should check your cyrus/lmtp setup.
