[Web-cyradm] Database Errors in 0.5.5

I have been trying to get 0.5.5 working on my test system here and have 
encountered an issue. Everytime I try to create a new domain on the system, 
I get a message on the screen that states "DATABASE ERROR, PLEASE TRY 
AGAIN."

This setup was working perfectly with 0.5.4. This is NOT an upgrade, this is 
a fresh install. My MySQL settings are all correct, the database was created 
just fine and populated using the bundled scripts.

Any ideas on where to go from here?
